The BSCAT™ method refers to a method that links modern risk–based safety management approaches to systematic root cause incident investigation. TheBSCAT™methodreferstoamethodthatlinksmodernrisk–basedsafetymanagementapproachestosystematicrootcauseincidentinvestigation.The“B”referstobarrier–basedaseachbarrieridentifiedinbowtieriskassessmentsistestedforwhyitfailed. SCAT™isSystematicCauseAnalysisTechnique,awell-establishedrootcauseanalysisapproachwhichincorporatestheDNVlosscausationmodel.Themodelisasequenceofdominosestablishingthehierarchyofaccidentprogressionfromtheimmediatecausebacktofundamentalrootcausesandsystemfailures. BSCATMethod Inshort-BSCATisthebarrierbasedextensiontoDNV’sSCATmethod. TheSCATmodel(SystematicCauseAnalysisTechnique)wasdevelopedtohelpincidentinvestigatorsapplytheDNVlosscausationmodeltoactualevents.ThisisdonebymeansoftheSCATchart.Thechartwascreatedtobuild–outaneventusingstandardizedeventdescriptionsthatcanfitthewholerangeofincidentsandnearmisses.Duetousingastandardizedlist,incidentanalysesaresuitabletoaggregation,leadingtomoreinsightintotheweakareasofyoursafetymanagementsystem. Abarrier-basedaccidentinvestigationstillappliestheSCATmodelbutnowitisappliedtoeachbarrierseparately,nottotheincidentasawhole. TheBSCATchartisthelatestupdateoftheSCATapproach.Itincludesallthehistoricaloccupationalsafetytopicsandunderlyingcauses,buthasaddedprocessriskstothelist(e.g.firesandexplosions)andingeneralmoresuitabletobarrieranalysis. TheBSCATsoftwareallowsyoutoreuseandlinkexistingriskassessmentinformation(bowties)anddofullintegrationofincidentanalysisandriskanalysis. Ifapplicablebowtiediagramsareavailableforuseduringyourinvestigation,youcanbringeventsandbarriersfromthebowtiedirectlyintoyourBSCATanalysis.Thisresultsinabetterfitbetweenincidentandriskassessmentanalysis,whichinturnallowsyoutoimprovetheriskassessment. Themismatchbetweenriskanalysisandincidentanalysisreferredtoabove,isusuallyintheperspective/abstractionlevelofthedefinedbarriers–barriersinincidentanalysestendtobedescribedtoospecifically,andtheyareless‘mappable’ontothebarriersinyourriskassessments. Byreusingthebowtieriskanalysisand/ordescribingthebarriersinyourincidentanalysesatthesameabstractionlevel,thisgapisbridgedandmorevalueisextractedfromyourincidentanalyses. Thisentireprocessallowsyoutogaugebarriereffectivenessandavailabilitybasedonreal-worldinformationextractedfromtheincidentanalyses. BackToTop
