Abstract Usuallyinthesubtotalgastrectomy,theleftandtherightgastricarteries,aswellastheleftandtherightgastroepiploicarteriesareligated.Thus,toavoidabluestomachsurgeonspreservethespleenandtheshortgastricarteries.Whenaradicalsubtotalgastrectomywithsplenectomyisperformed,meticulouscautionisnecessary;otherwise,thesubtotalgastrectomymighthavetobechangedtoatotalgastrectomytopreventabluestomach.Wereportthecaseofa67-year-oldwomanwhohaddistalstomachcancerwithasplenicsolitarymass,forwhichsplenicmeatastasiscouldbeexcluded.Weplannedandperformedalaparoscopy-assistedradicalsubtotalgastrectomywithsplenectomyasthediagnosticandtherapeuticoption.Inthiscase,toavoidaremnantstomachinfarctionortotalgastrectomywesavedtheleftgastricarteryandveinwithclearingperivascularsofttissue,lymphatics,andlymphnodes.Thustheradicaltherapeuticgoalwasreached,andseriouscomplicationswereavoided.
