RetiredteacherDudleyDragonhadaheartbypasslastJuly,butyouwouldnotknowitfromlookingathischest.Insteadofthelong'zipper'scarthatmarksoutheartbypasssurvivors,allhehasaretwo2cmlongscarsanda5cmlongone.HissurgeonsattheNationalHeartCentre(NHC)usedarobottoperformminimallyinvasive'keyhole'surgery.Thatmeanthewassparedfromhavingtohavehisbreastbonesawnopen-apainfulfeatureofconventionalbypasssurgery.Hisdoctorscouldalsooperatewhilehisheartwasbeating,soitdidnotneedtobestoppedandrestarted.MrDragon,80,whowenthomeafterfivedaysinhospital,said:'Iwassparedallthat,Iwassolucky.Surprisingly,therewasn'tanypainaftertheoperation.'Theuseoftherobot,calledthedaVincisurgicalsystem,hasallowedheartsurgeonsheretoperformheartbypasses,repairheartvalvesandremovechesttumoursinaminimallyinvasiveway.Sofar,theyhavecarriedoutoperationson15patients,includingMrDragon.MountElizabethHospitalhasdoneahandfulofroboticheartprocedurestoo.Asroboticheartproceduresarestillnew,doctorsoperateonlyonpatientswhoseconditionsarenotcomplicated,andwhoagreetotheprocedures.MrDragonrecalledmakingthedecision:'AtfirstIwaspuzzled,andaskedmydoctor:'Howcantherobotdotheoperation?''Butafterhesaidhecontrolsit,andthathehadoperatedonfourpatientssuccessfully,Iwasreassured.'TheNHCstartedathree-yearprogrammein2006toevaluateroboticheartsurgery,fundedby$1.5millionfromitsparentgroup,SingaporeHealthServices.Thetrialwillbeconsideredsuccessfuliftheresultsare'atleastequivalenttoconventionalprocedures',saidAssociateProfessorKohTianHai,theNHC'smedicaldirector.Theroboticprocedureismeanttoresultinlesspain,lessbloodlossandfasterrecoveryforpatients,saidDrC.Sivathasan,whoisleadingtheroboticheartsurgeryprogramme.StudiesintheUnitedStatesshowthatmostpatientsaredischargedtwotothreedaysaftertheoperation,whereasnormalbypasspatientsusuallystaysixorsevendaysinhospital.Mostpatientsherearedischargedafterfourtosixdays,asdoctorswanttobecareful,DrSivathasansaid.Inconventionalbypasssurgery,surgeonsneedtoopenupthechesttoreachtheheart.Theyhookupthepatienttoaheart-lungmachine,whichtakesoverthefunctionsofthoseorgans,andstoptheheart.Theythenattachavein,removedearlierfromthepatient'sthighorchest,totheheartarteriestocreateabypassthatre-channelsbloodawayfromblockages.Inaroboticprocedure,surgeonsnolongerneedtocutopenthechestbecausetherobot's'arms'candothejobwithinthechest,enteringthebodythroughsmallcuts.Seatedataconsole,themainsurgeoncontrolstheroboticarmstogettheworkdone.Fornow,theNHCchargesthesamefeeforroboticproceduresasconventionaloperations,thougheachroboticprocedurecostsfrom$2,500to$5,000more.Anormalbypasstherecostsapatientstayinginaprivatewardabout$24,400.Therobothasalsobeenusedforprostatesurgeryheresince2003.TheNHCmarksits10thanniversarythisyearwithadinnertomorrow.Heartfacts•Whyitmatters AdvancesinhearttreatmentaffectalargenumberofpeopleinSingapore.HeartdiseaseistheNo.2killeraftercancer,accountingforalmost23percentofdeathshere.•Whohasbypasssurgery?Surgeonsperformabout4,600open-heartoperations-mostlybypasssurgery-inSingaporeeachyear.•Whatitcosts Apatientinaprivatewardpaysabout$24,400andstaysinhospitalforaboutninedaysafteropen-heartbypasssurgeryattheNationalHeartCentre.Fornow,robotic-assistedsurgerypatientspaythesame,thoughtheheartcentresaystheiroperationscost$2,500to$5,000morethannormalbypasssurgery.
