Mania A“manicstate”or“manicepisode”isastateofelatedorirritablemoodthatispresentthroughoutthedayforatleastaweekandcanbeviewedbyemotion,thought,attitude,movement,speech,behavior,andphysicalstate: Emotion:Theyareconsciouslyhappy,excited,relaxed,cheerful,oftenoverjoyedfornoreason,occasionallyveryirritable,angry. thought:Theyhaveinflatedorexaggeratedself-esteem,believingtheyhavesuperpower,greatplans,wealth,status,andboastingofbeingastarorgod,accompaniedbyflightofideaoroverwhelmingfeelings. Attitude:Theybecomenosy,bold,overconfidentandgenerous. Movement:Theyarewalkingaroundallday,doingthingsallthetime,andhavedecreasedsleepneeds(e.g.sleepingforonly3hoursisenoughforthem). Speech:Theyareuncharacteristicallytalkingnon-stoptotheextentthattheirvoiceishoarse. Behavior:Theyarespendingmoney,donatingmoney,investinglikemad,doingunusualandunwisethings. Physicalstate:Originallyshy,introvertedandreticent,theysuddenlybecomeveryenthusiasticandeagertopursuetheoppositesex.Sometimestheyaresoexcitedthattheycouldnotsleepatnight. Depression A"depressivestate"or"depressiveepisode"referstothestateofbeingextremelydepressedandunhappy,andlosinginterestorpleasureinallactivitiesforatleast2weeks.Itcanbeviewedfromemotion,Thought,attitude,speech,movementandphysicalstate: Emotion:Theyaredepressed,listlessandsadallday. Thought:Theybecomepessimistic,negative,sad,andevenfeelthattheyhavecommittedaheinouscrimeandthattheydeservetodie,orfeelthattheyhaveaserious,incurabledisease,andmayhavesuicidalthoughtsorsuicidalbehavior. Attitude:Alldaylong,itisobviousthattheyarenotinterestedineverything.Theyareextremelypessimisticandnegative.Theirconcentrationdropssignificantlyandtheyarehesitanttodealwiththings. Speech:Theybecomesilent,andwhentheydospeak,theyspeakveryslowlyorwithpauses,ofteninalowvoicethatmakesithardtohearwhattheyaresaying. Movement:Theymoveslowly,oftensitinadaze,sleepinbedallday,feellethargicallday,sleeplessatnight. Physicalstate:Theyhavesignificantweightloss,poorappetite,constipation,aflagginglibido,impotenceinmen,cessationofmenstruationinwomen,andphysicalfatigueandleadenfeelinginlimbsthroughouttheday. Treatmentforbipolardisorder Pharmacotherapy Manystudieshavesuggestedthatthephenomenonofbipolardisordermaybeassociatedwiththedysfunctionoftheneurotransmitterbetweencertainnervecellsinthebrain.Therefore,treatmentofmaniaismainlybasedonmoodstabilizers,suchaslithium. Inthecaseofacutemania,antipsychoticsarecombinedtocontroltheagitation,whileantidepressantsareusedforthetreatmentofdepression,anditusuallytakes2-3weeksforthetreatmenttobeeffective. Non-pharmacotherapy Psychologicalandfamilytherapy. Somestudiessuggestthatlifeeventsandpsychologicalconflictsmayplayaroleintheonsetofthedisorder.Therefore,whendiseasecontrolreachesremission,psychologicalandfamilytherapycanbecombinedtopreventrecurrenceofthedisease,toearlydetectthesignsofrecurrence,toassistwithpersonaladaptation,familymembercareandfinancialburdenmanagement.
